After you submit: You'll be redirected to Step 2 to open the BGMD Team Portal and complete onboarding.

This form collects the information we need to set up your BGMD account, email, Microsoft Teams access, and phone extension for internal communication.
Information in this section is not shared publicly and is used only for internal operations and account setup.
